ORDERING PROCEDURE

For a combined paper and online subscription, or for an "online only" subscription, send payment to:

Once the Journal has received payment, you will receive a letter with instructions for obtaining access to the World Wide Web edition; the letter will direct you to an online form. You will be asked to provide the following information necessary to activate your subscription to JHC Online:

A user name and password that you can easily remember

Your name, phone number, and email address

Your customer number (included with the letter, this alpha-numeric string also appears on each mailing label for the print copy (Ex: A1234567)
